| PAYMENT TERMS AND CONDITIONS |
| Payment forms accepted: BANK TRANSFER/ CHEQUE / CASH ( AND CARD – DR LAKHERA) PAYEES: – Dr M B Speight and Mrs L Speight LLP (for patients seeing Dr Speight) or – Dr R Lakhera MSK (for patients seeing Dr Lakhera) Please note that patients are expected to settle payment for each appointment before a further appointment can go ahead. |
| 1) All patients will need a recent consultation with our doctor before they can perform a procedure. This is to ensure that any treatment recommended is appropriate for your condition. If you have a new condition, or a condition that our doctor has not seen you with for more than a year, you may require an initial consultation again 2) To receive the discounted self funding rates, payment is to be made at the time of the appointment or within 7 days of appointment if paying by bank transfer. Otherwise, an additional charge will be made of £15.00 per appointment. Any additional discount offered is also subject to payment terms of 7 days. In some instances, discretionary funding may be provided by other third parties (such as a specific occupational scheme), but where this is the case, patients may still need to self fund and claim eligible costs back from the relevant third party. For this reason we would advise patients to obtain any agreement for discretionary funding in writing from their third party. For patients who wish to claim back from a “Hospital Fund” or “Cash Plan” policy such as those operated by Simply Health, please ensure that you check the terms of your policy before you attend. Most of these policies will cover therapies such as physiotherapy, podiatry and osteopathy. As far as medical claims on these policies are concerned, the General Medical Council (GMC) does not have a specialist register for Musculoskeletal Medicine which means that under the terms of most cash plans, you will not be covered for our medical services. Some private medical insurers do have discretionary recognition for specialist doctors in Musculoskeletal Medicine, but they do not tend to apply these criteria to their simpler cash plan policies. |
| 4) OVERDUE INVOICES: For accounts which still have not been settled within 30 days of the appointment date, we reserve the right to add administration costs and interest as follows: • Reminder invoice administration charges: £5.00 per reminder invoice sent • On outstanding accounts referred to debt collection, there will be an administration fee of £50.00 or 30% of the outstanding amount (whichever is greater). • Interest may also be charged at a rate of 8% above the Bank of England base rate. |
| 5) MISSED APPOINTMENTS AND LATE NOTICE CANCELLATIONS If you need to change or cancel your appointment, you must give us a minimum notice of one full working day (minimum 24 hours). Please note that we do not class weekends and Bank Holidays as working days. For late cancellations or missed appointments, practitioners are entitled to charge in full for the appointment time, although this may be reduced or waived depending upon individual circumstances at the practitioner’s discretion. |
